MANAJEMEN FISIOTERAPI PADA XEROSIS, NYERI DAN KELEMAHAN OTOT DENGAN MODALITAS OILING, INFRA RED DAN TERAPI LATIHAN PADA KASUS KUSTA TIPE MULTIBASILER (A Case Report)

Kusta atau leprosy adalah penyakit infeksi kronis yang disebabkan Mycobacterium leprae, dan hampir mirip seperti penyakit tuberculosis. Kondisi ini menyerang saraf sensoris, motoris dan perifer. Manifestasi klinik dapat berupa kulit kering atau xerosis, nyeri, dan kelemahan otot. Program fisioterapi yang dapat diberikan bertujuan untuk mencegah, memperbaiki dan meminimalisir deformitas. Program tersebut terdiri dari oiling, infra red dan terapi latihan. Tujuan: Untuk mengetahui manajemen fisioterapi pada xerosis, nyeri dan kelemahan otot dengan modalitas oiling, infra red, dan terapi latihan pada kasus kusta tipe multibasiler. Metode: Studi kasus ini dilakukan pada seorang pasien kusta di RS Kelet Donorejo. Problematika yang dialami pasien adalah xerosis, nyeri dan kelemahan otot tungkai bawah. Hasil: Pemeriksaan elastisitas kulit dengan ODSS menunjukkan peningkatan elastistas 1 tingkat dari nilai 3 menjadi 2. Pemeriksaan nyeri dengan menggunakan NRS (Numeric Rating Scale) menunjukkan penurunan nyeri diam dari nilai 4 menjadi 1, nyeri gerak dari 3 menjadi 1, dan nyeri tekan dari 2 menjadi 0. Kekuatan otot diukur dengan MMT (Manual Muscle Testing) menunjukkan peningkatan kekuatan otot plantar fleksor, dorsal fleksor, invertor dan evertor dari nilai 4 menjadi 5, sedangkan untuk kemampuan fungsional diukur dengan FADI (Foot and Disability Index) menunjukkan peningkatan kemampuan aktifitas fungsional dari nilai 3 menjadi 4. Kesimpulan: Intervensi oiling, infra red dan terapi latihan mampu mengurangi xerosis dan meningkatkan kekuatan otot tungkai bawah.
